Prepare For Your First Appointment
We ask that you arrive a few minutes early to your first appointment. You can save some time by completing the patient registration forms online ahead of time.
What to Bring
We ask that you bring the following items to your first appointment:
- Referral form
- X-rays or CT scans on a CD
- A list of current medications
- Photo ID (e.g., driver’s license)
- Insurance information
Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian during the consultation appointment.
What to Expect
Your first visit with us is a great time to ask any questions you have about oral surgery, learn what to expect, and explore your treatment options. You will be meeting with our board-certified surgeon, Dr. Foxson, who will explain your anesthesia options for treatment and discuss the ways that we can improve your oral health. We believe that keeping our patients educated and informed is a great way to increase comfort levels. Our office is equipped with state-of-the-art 3D CBCT scanning technology, which helps us get a clear view of your condition.
Financial and Insurance Information
Our front desk staff will try to help you get the most out of your insurance reimbursement. We provide exceptional services at a reasonable cost to our patients. Please remember that payment is due at the time service is rendered unless prearranged agreements have been made with our staff. Most insurance companies will respond within 4 to 6 weeks. Any remaining balance is your responsibility, and you are fully responsible for all fees charged by this office regardless of insurance coverage. We are happy to make payment plan arrangements, but this must be discussed and approved prior to your procedure.
